Dementia cases ascertained as part of study: At *Baseline* 51 (3%) participants had formal diagnosis of dementia; n=13 participants had mild cognitive impairment (memory clinic ascertainment). At *follow up* we did not undertake clinical examinations to define incident dementia, but 179 individuals had Z scores less than –2·0 from the baseline assessment.
